I and all beings extensive as space, my mothers, take refuge in the Lama, the precious Buddha.
We take refuge in the Buddha, Dharma and Sangha.
We take refuge in the Lama, Yidam and Dakini and their assemblies.
We take refuge in our own mind - empty and luminous - the dharmakaya. Repeat as much as you can, and then:

In Buddha, Dharma and Sangha most excellent, I take refuge until enlightenment.
By the merit of practicing generosity and so on, may I attain buddhahood in order to benefit beings. (3 times)

Generating the Deity:  

Above the crown of my head and that of all beings pervading space, on a white lotus and moon disk, from HRIH appears the supreme Phakpa Chenrezik,brilliant white and radiating five-coloured lights, beautiful and smiling, gazing with compassionate eyes.

Of his four hands, the first two are joined in devotion; the lower two hold a crystal rosary and a white lotus. He is adorned with silks and jewel ornaments, and wears an upper robe of doeskin.His crown ornament is Amitabha. He sits with his legs crossed in vajra posture, resting his back against a stainless moon. He is the embodiment of all objects of refuge.

Think that you and all beings are making the following prayer with one voice:

Lord of whitest form, unstained by faults, whose head is adorned with a perfect buddha, and who looks uponbeings with compassionate eyes: Chenrezik, to you I bow down. Repeat three, seven, or as many times as you can.

The Supplication of Calling with Longing (by Gelong Pema Karpo):

I supplicate you, Lama Chenrezik. I supplicate you, Yidam Chenrezik. I supplicate you, supreme Phakpa Chenrezik. I supplicate you, lord of refuge Chenrezik. I supplicate you, loving protector Chenrezik.

Hold us in your compassion, compassionate victorious one. For beings who have wandered through countless eons in endless samsara and experience unbearable suffering, there is no other refuge but you, lord. Grant your blessing that they may achieve omniscient buddhahood.

  Because of accumulating negative karma from beginningless time, due to aggression, sentient beings are born in the hells and experience the suffering of heat and cold. May they be born in your presence, supreme deity. OM MANI PEME HUNG.

Because of accumulating negative karma from beginningless time, due to miserliness, sentient beings are born in the hungry ghost realm and experience the suffering of hunger and thirst. May they be born in your supreme pure field of Potala. OM MANI PEME HUNG.

Because of accumulating negative karma from beginningless time, due to bewilderment, sentient beings are born as animals and experience the sufferings of stupidity and dullness. May they be born in your presence,
protector. OM MANI PEME HUNG

  Because of accumulating negative karma from beginningless time, due to desire, sentient beings are born in the human realm and experience the sufferings of constant toil and poverty. May they be born in your supreme pure field of Dewachen. OM MANI PEME HUNG.

Because of accumulating negative karma from beginningless time, due to jealousy, sentient beings are born in the realm of demi-gods and experience the suffering of fighting and quarreling. May they be born in your pure field of Potala. OM MANI PEME HUNG.

Because of accumulating negative karma from beginningless time, due to pride, sentient beings are born in the realm of the gods and experience the suffering of falling to the lower realms. May they be born in your pure field of Potala. OM MANI PEME HUNG.

Wherever I am born, birth after birth, by activity equal to Chenrezik, may I liberate all beings of the impure realms, and may the supreme speech of your six syllables pervade throughout the ten directions.

By the power of supplicating you, Supreme Victorious One, may beings to be tamed be mindful of karma and its results and apply themselves to virtuous actions. May they act in harmony with dharma for the benefit of beings.

When I have prayed this way one-pointedly, lights radiate from the form of Phakpa Chenrezik and purify all impure karmic appearances and confused consciousness. The outer world becomes Dewachen, the pure field of bliss; body, speech, and mind of beings within become Chenrezik's perfect Body, Speech and Mind -
Appearance, sound and awareness are inseparable from emptiness.

OM MANI PEME HUNG HRIH Recite the mantra as much as you can. At the end let the mind rest evenly in its own nature, without conceptualizing the three spheres (subject, object and act).

I and all others appear as the form of Chenrezik. All sounds are the melody of the six syllables. All that arises in the mind is the expanse of primordial awareness.

  By this merit, may I quickly achieve the state of mighty Chenrezik, and establish every being, without exception, in the same enlightened state.
(This bears the blessing of the great Siddha Thangtong Gyalpo's Speech.)

By the merit of this meditation and recitation, may I and all beings with whom I have a connection,
as soon as we leave behind this impure body, be miraculously born in Dewachen. As soon as we are born there, may we traverse the ten bodhisatva stages and through our emanations accomplish the benefit of all beings in the ten directions.

I dedicate all this merit that I may follow in the footsteps of the heroic Manjushri who knows the truth of the essential nature of things, and of Samantabhadra, too. By this merit, may all beings fully accumulate merit and wisdom, so they may obtain the two kayas of Buddha that spring from these two accumulations.

By the blessing of the Buddha having achieved the three kayas, by the blessing of the truth of the Dharma itself being unchanging, and by the blessing of the wishes of the Sangha beings unwavering, may this dedication prayer be fulfilled. Seal the practice with these and other dedication and aspiration prayers.

here is the Prayer that is Famous for its Blessing of Removing Sickness at Sakya:

This Refuge was transmitted by Phakpa Chenrezik to Ka Ngapa Paljor Sherab, and he transmitted it to the Great Mahasiddha
Thangtong Gyalpo, who then performed it for the benefit of the infinite sentient beings:

I and all beings extensive as space, take refuge in the Lama, the precious Buddha.
We take refuge in the Buddha, Dharma and Sangha.
We take refuge in the Lama, Yidam and Dakini and their assemblies.
We take refuge in our own mind - empty and luminous - the dharmakaya. Repeat this as much as possible, and then:

OM MANI PEME HUNG one hundred or more times.

All harmful negative forces, sicknesses, devils and so on, that come from karma, accidents and adventitious circumstances, all the illnesses from sentient beings' unhappy minds: in the whole world, may none of these arise.

Like being led away by an executioner, who separates body and mind in an instant, so, all the miseries of sickness that steal away one's life-force - in the whole world, may none of them arise. Trapped in the mouth of the lord of death, Yama, even just hearing the name of a disease causes fear.

From all these deadly diseases - those that kill in one day, those that are long-lasting, etc. - may no harm come to any sentient being.

The eighty thousand kinds of harmful obstructors, the three hundred and sixty kinds of sudden and adventitious accidents, the four hundred and four kinds of disease and so on: may none of these harm any sentient being. May all suffering from disturbances of the four elements, which steal away one's physical and mental wellbeing and happiness, be completely pacified: may everyone have the healthy glow of vitality and strength, and enjoy a long and happy life free from sickness.

By the compassion of the Lamas and the Three Jewels, by the powerful ability of the Dakinis, Dharma Protectors and Guardians, and by the strength of the non-deceptive truth of cause and effect, may my authentic dedication and aspiration prayer be fulfilled.

At the Seat of the Great and Glorious Sakya, during a rampant epidemic, when neither sutra, torma, medicine, mantra protection nor any other practices the yogi priests applied had any effect, and the Seat (of Sakya) was almost empty, the Great Lord, the Mahasiddha (Thangtong Gyalpo) said: "practice this Prayer with the Refuge 'Manamka' and counting Mani mantras". When this was done according to his command, all sickness was immediately stopped.

Therefore, this Prayer called "The Sakya Prayer that Removes Illness" from his Vajra Speech is famous for emanating clouds of blessings. Mangalam.

OM MANI PEME HUNG

King of Great Loving Kindness, supreme Phakpa Chenrezik, King of Wrathful Ones, Hayagriva, Jetsün Drölma and others, just hearing your names dispels all fears. Sources of Refuge, quintessence of compassion, consider me:

In this time of strife of the five rampant degenerations, when (the water from) the great lake of evil actions and jealousy gushes out, and beings are tormented by terrible suffering from fighting and quarreling, please dry it up by the fire of your wisdom and compassion.

Upon the beings who burn with the fires of hatred, let fall a great rain of the nectar of loving kindness, and bless them to be calm and peaceful, and to have loving concern for each other, like parents.

In whoever's mind a devil has entered, causing them to become demigod-like, (i.e. consumed by jealousy, engaging in constant warfare) eliminate all these devils completely, so they can never come to this area again.

May all sentient beings who have died in war and fighting henceforth renounce all bad actions and their cause and effect, and be born miraculously in Dewachen, the Realm of Bliss: please lead everyone to that Buddha-field.

May all beings have a long life free from sickness and, with all fighting and quarreling completely pacified, may they engage only in the ten virtues.

May the rains fall at the right time, the harvests be excellent, and the animal herds stay healthy always. Please grant your blessing for goodness to flourish and increase forever, for all beings in all the worlds.

By the dharmata which is by nature perfectly pure, and non-deceptive cause and effect regarding (conditioned) phenomena, and by the compassion of the Lamas, Yidams and Three Jewels, may this completely pure, expansive wishing prayer be fulfilled.

When in the Minyag region of Kham the continuous conflicts and fighting could not be stopped, and and no one who tried was able to resolve them, at that time the Great Lord, the Mahasiddha (Thangtong Gyalpo) went there, feeling great compassion for them.

After speaking the Words of Truth, by merely scattering some flowers, all malevolent thoughts of jealousy and hatred in everyone's mind were pacified, and the conflicts and fighting stopped for good; unusually abundant crops and marvelous occurrences - every kind of goodness arose. Therefore his Vajra Speech has great blessings.

From the Enlightened Speech of the Great Mahasiddha Thangtong Gyalpo,

here is the Prayer that Completely Pacifies the Fear of Famine:

In order to definitely liberate the limitless sentient beings, you gave rise to the mind of supreme unsurpassed enlightenment (bodhichitta) and completed the two accumulations:

Compassionate Victors, Buddhas of the ten directions, Bodhisatvas, Shravakas, Pratyekabuddhas and hosts of Lamas, Yidams, Dakinis and Dharma Protectors, with your entourage of Yakshas, Wealth Gods and Treasure Guardians, consider with your love all the beings who in this time of degeneration have no protector, and are weakened by constant suffering from hunger and thirst.

In order to be a refuge for all these beings, shower down upon the world [lit., Land of Snow] a rain of precious things such as food, gems, grains, homes, clothes and so on - all that is pleasing and necessary for one's use and enjoyment.

May untimely windstorms, fires, droughts, floods and so on, any harm from the four elements, be completely pacified, and may nutritious grains, flowers, grasses, crops, harvests and so on, ripen and increase perfectly like during the era of the Golden Age.

Through the power of this great prayer of aspiration rising from the energy of my great love and compassion, may all misfortune of sickness, war and famine be completely pacified. May all have a long and healthy life, full of happiness and joy.

By the compassion of the Buddhas and their Sons, and by the strength of the non-deceiving truth of cause and effect, may the Buddha's Teachings flourish and spread, and may all beings quickly attain unsurpassed Buddhahood.

At a time when in Ü and Tsang great fears of a famine arose because of a bad harvest due to frost, hail etc., the Great Lord and Mahasiddha, as an offering, filled the golden alms bowl of the Jowo Rinpoche with grain and made this wishing prayer from his heart.

All those who had a karmic connection with him saw Phakpa Chenrezik showering down a rain of grain from the sky all over Tibet, and by this auspicious occurrence, all beings were saved from the suffering of hunger and thirst. Therefore his Vajra Words have great blessings.
